To support this technical review, a comprehensive search of electronic databases (including MEDLINE, the Cochrane Database of Systematic Reviews Database of Abstracts of Reviews of Effectiveness, American College of Physicians Journal Club, British Medical Journal Clinical Evidence, EMB Reviews, CINAHL, EMBASE, and HealthSTAR) was performed by a professional evidencebased medicine company to identify relevant articles from 1990 to 2003. The search was restricted to articles involving human studies that were available in English. Additional relevant articles published after the search was completed that were identified by the authors were also included.
