Clinical correlation between Child - Pugh classification, the blood level of β2 - microglobulin and arterial oxygen in liver cirrhosis patients

摘要: 目的探讨肝硬化患者血清β2-微球蛋白(β2-MG)及动脉血氧变化与Child分级的关系。方法选取肝硬化患者58例,应用放射免疫法(RIA)检测血清β2-MG水平,应用血气分析仪测定动脉血氧分压(PaO2),并与30例正常献血员血清β2-MG、PaO2水平进行比较。结果肝硬化组血清β2-MG、Child B级高于A级,Child C级明显高于B级,差异有统计学意义;动脉血氧分压从A—B—C级间逐渐下降,低氧血症发生率逐渐升高,Child C级、B级间比较差异有统计学意义。结论提示血清β2-MG与肝功能的损害程度关系密切,PaO2则随着肝功能损害程度加重而逐渐下降,在临床上对肝硬化患者的病情估计及预后判断有一定的指导意义。

Abstract: Objective To investigate the correlation between Child-Pugh Classification and, the blood levels of β2-mi-croglobulin and Arterial Oxygen in Patients with Liver Cirrhosis.Methods Fifty eight liver cirrhotic patients and 30 healthy controls were included in our study.Blood β2-microglobulin and partial pressure of oxygen in arterial blood were detected by radioimmunoassay and automatic blood gas analyzer, respectively.Results The Blood β2-microglobulin level in cirrhotic patients of Child B was significantly higher than that of child A and patients of Child C level was higher than that of child B.Arterial partial pressure of oxygen and the rate of hypoxemia were gradually decreased and increased in the pa-tients from child A, respectively.Conclusion The blood level of β2-microglobulin is associated with the severity of liver damage.The arterial partial pressure of oxygen gradually decreased with the aggravation of liver damage.The combined blood β2-microglobulin and arterial partial pressure of oxygen could be considered for the evaluation and prognosis of liver damage.-
