Effects of myeloperoxidase on the pathogenesis of acute pancreatitis

摘要: 目的通过测定43例急性胰腺炎(AP)患者血清髓过氧化物(MPO)水平与细胞因子水平,并施与己酮可可碱治疗,研究血清MPO水平与细胞因子水平及胰腺炎严重程度、时程的关系,确定MPO在AP中的作用,探索AP治疗新途径。方法选取43例AP患者(24例轻症和19例重症)和18例对照者,分别入选试验组与对照组。抽取所有患者入院时及入院后1、2、3、7、14天的静脉血标本。通过RIA法测定血清MPO水平,通过ELISA法测定血清IL-6、IL-8、TNF-α水平。结果患者血清MPO水平与IL-6、IL-8、TNF-α水平呈正相关。重症胰腺炎患者血清MPO最高水平出现在病程第一天,并且MPO水平于第三天下降。己酮可可碱可显著降低血清MPO水平。结论血清MPO水平与血清细胞因子水平和AP的严重程度呈正相关。己酮可可碱应用在重症急性胰腺炎综合治疗中可提高治疗效果。Abstract: Objective To study the relationship between serum MPO levels, cytokine level and severity in acute pancreati-tis (AP) patients and to explore a new therapeutic method.Methods Forty three patients with AP (24 patients had mild and 19 had severe pancreatitis) and 18 healthy controls were included in the study.The serum level of MPO and, IL-6, IL-8 and TNF-α were measured by MPO-EIA and ELISA, respectively on days 1, 2, 3, 7, and 14.Results The MPO level was positively correlated with the levels of proinflammatory cytokines (IL-6, IL-8 and TNF-α) in AP pa-tients.The highest level of MPO in patients with severe AP was observed at the first day and decreased at the third day.Administration of pentoxifylline significantly reduced the blood MPO level.Conclusion The MPO blood level positively correlates with the cytokine levels and the severity of AP.Pentoxifylline therapy may improve severe AP condition.
