摘要: 目的研究胆囊腺癌、癌旁组织和慢性胆囊炎组织中的α-甲酰基辅酶A消旋酶(P504S)和P63蛋白表达及其临床意义。方法108例胆囊腺癌、46例癌旁组织和35例慢性胆囊炎手术切除标本常规制作石蜡包埋切片,P504S和P36染色方法为EnVisionTM免疫组化法。结果胆囊腺癌P504S和P63阳性率(51.9%,43.5%)及其评分(2.13±1.88,1.98±1.73)明显高于癌旁组织阳性率(17.9%,10.9%)及其评分(0.72±1.24,0.61±1.39)和慢性胆囊炎阳性率(14.3%,6.0%)及其评分(0.59±1.34,0.32±1.22)(P<0.01);腺瘤癌变或高分化腺癌、无淋巴结转移及未侵犯周围组织器官病例P504S表达阳性率及其评分明显低于低分化腺癌、淋巴结转移及侵犯周围组织器官病例(P<0.05,或P<0.01);无淋巴结转移病例P63表达阳性率及其评分明显低于转移病例(P<0.05)。结论P504S和P63表达与胆囊腺癌发生发展有密切关系,P504S可能是反映胆囊腺癌生物学行为和预后的重要生物学指标。

Abstract: Objective To study the expressive levels of P504S and P63 and detect their clinicopathological significances in gallbladder adenocarcinoma, pericancerous tissues and chronic cholecystitis.Methods Immunohistochemical method of EnVisionTM was used for assaying the expressive levels of P504S and P63 in routinely paraffin-embedded sections of the specimens from patients with gallbladder adenocarcinoma (n=108) , pericancerous tissues (n=46) and chronic cholecystitis (n=35) .Results The positive rates and scores of P504S and P63 in gallbladder adenocar-cinoma were significantly higher than those in pericancerous tissues and chronic cholecystitis (P<0.01) .The positive rates and scores of P504S was significantly lower in the cases of adenomatous canceration or high-differentiated adenocarcinoma, no-metastasis of lymph node and no-invasion of regional tissues than those in the ones of low-differentiated adenocarcinoma, metastasis of lymph node and invasion of regional tissues (P<0.05 or P<0.01) .The positive rate and score of P63 was significantly lower in no-metastasis of lymph node than that in metastasis of lymph node (P<0.05) .Conclusion The expression of P504S or P63 might be related to the carcinogenesis and progression of gallbladder adenocarcinoma, and P504S might be important biological marker for reflecting the biological behaviors and prognosis of gallbladder adenocarcinoma.-
