Research advances in autoimmune hepatitis-related hepatocellular carcinoma

  • 摘要: 自身免疫性肝炎(AIH)相关肝细胞癌(HCC)是指在长期AIH的基础上发生的HCC,其发病率较低(0~6%)。高龄、男性、糖尿病、酒精使用、AIH反复复发与持续ALT异常、免疫抑制治疗和治疗失败及长期肝硬化状态等可能是AIH患者发生HCC的危险因素。肝硬化是AIH发生HCC的重要阶段。AIH进展到肝硬化后,HCC发病率显著升高。目前国内外关于AIH发生HCC的机制鲜有报道,可能与肝硬化诱导下特异性分子生物学特征(染色体、端粒、基因等)的改变、细胞死亡-炎症-癌症途径及肠道微生态紊乱等有关。及时识别AIH可能发展为HCC的高危人群并加强干预、随访和监测非常重要。
