<正>1病例资料患者女性,56岁。最初于2016年诊断为急性胰腺炎（acute pancreatitis,AP）,予内科对症治疗后,腹痛好转出院。出院后严格控制饮食,未暴饮暴食及食用油腻食品。2019年4月因"中上腹痛不适20 d,加重1 d"入院,查淀粉酶443 U/L,脂肪酶698 U/L,血常规、血脂、肝肾功能均正常。磁共振胰胆管造影（MRCP）示:胆囊结石,胆囊炎;胰腺炎改变,胰周渗出,胰管扩张胰尾部囊性信号影;胆总管稍扩张,胆总管下段小结石可能;肝左叶、左肾异常信号（图1a）。诊断为急性胰腺炎（胆源性）,胆囊结石伴胆囊炎,胆总管结石。行腹腔镜胆囊切除术+十二指肠镜检查术+十二指肠乳头息肉活检术,
