The change and its clinical significance of levels of endotoxin and cytokines in cirrhotic patients with ascites.

摘要: 探讨肝硬化腹水患者体内前炎症细胞因子TNF-α、IL-6及内毒素水平的变化、相互关系及临床意义。对45例患者的腹水、血液标本,采用鲎试剂三肽显色基质偶氮法检测内毒素水平,双抗体夹心酶联免疫吸附法检测TNF-α、IL-6水平。1.自发性细菌性腹膜炎（SBP）组、无菌性腹水（SA）组和健康对照组血浆内毒素水平分别为（0.265±0.010）U/L,（0.098±0.015）U/L,（0.029±0.003）U/L（P<0.05）;TNF-α分别为（1387.9±115.6）ng/L,（487.5±29.2）ng/L,（84.3±5.8）ng/L（P<0.05）;IL-6分别为（2149.1±160.9）ng/L,（639.7±42.6）ng/L,（94.7±8.5）ng/L（P<0.05）;2.SBP组、SA组腹水内毒素水平分别为（0.264±0.007）U/L,（0.055±0.009）U/L（P<0.05）;TNF-α分别为（1561.2±109.7）ng/L,（215.2±12.9）ng/L（P<0.05）;IL-6分别为（2571.0±268.1）ng...Abstract: To evaluate plasma and ascitic fluid levels of pro-inflammatory cytokines (TNF-α, IL-6) and endotoxin in cirrhotic ascitic patients.The plasma and ascitic fluid levels of endotoxin were assayed by limulus amebocyte lysate test and the levels of TNF-α, IL-6 were assayed by a double monoclonal sanwich enzyme linked immunosorbent assay (ELISA) in 45 cases of patients. (1) The plasma and ascitic levels of TNF-α, IL-6 and endotoxin were significantly higher in hepatitis cirrhofic patients with Spontaneous Bacterial Peritonitis (SBP) than those without SBP (P<0.05) .And they were higher in cirrhosis without SBP than those in healthy control group (P<0.05) . (2) The plasma level of endotoxin was significantly correlated with the level of TNF-α (r=0.857, P<0.001) and IL-6 (r=0.885, P<0.001) .It was also between the plasma level of TNF-α and Il-6 (r=0.957, P<0.001) . (1) Plasma endotoxin, TNF-α and IL-6 levels are increased in hepatitis cirrhosis patients with ascites. (2) Endotoxin may stimulate some kinds of immune activated cells to produce pro-inflammatory cytokines, TNF-α and Il-6, which may deteriorate the complications.
